PATIENT RESPONSIBILITIES

We feel you and your family are important in helping our team in your care and recovery. Please cooperate with the staff in your treatment and observe hospital regulations established for the mutual benefit of every patient.

Provision of information

You have the responsibility to provide, to the best of your knowledge, accurate and complete information about present complaints, past illnesses, hospitalizations, medications and other matters relating to your health. You have the responsibility to report unexpected changes in your condition (symptoms) to your physician or nurse.

It is important that you understand your responsibility in asking questions if you do not clearly understand any part of your treatment plan and what is expected of you.

Compliance with instructions

You are responsible for following the treatment plan recommended by the physician primarily responsible for your care. This may include following the instructions of nurses and health personnel as they carry out the coordinated plan of care, implement the responsible physician’s orders and enforce hospital rules and regulations. You are responsible for keeping appointments. When you are unable to do so for any reason, you are responsible for notifying the responsible physician or the hospital.

Treatment issues

You have the right to refuse treatment and to be informed of the medical consequences. However, you are responsible for your actions if you refuse treatment or do not follow the physician’s instructions. Likewise, it is your responsibility not to demand treatment or services that your physician has deemed medically unnecessary or inappropriate.

VISITOR POLICY AND PATIENT GUIDANCE

The Siddhasthali Rural Community Hospital visitor policy is highlighted below:

The visiting hours are restricted to the following time:
Morning: 07:00 am – 8:00 am
Day: 11:00 am- 12:00 pm
Evening: 04:00 pm-06:00 pm
It is mandatory to carry the visitor pass issued at the time of admission. The visitor shall vacate the room/ward during the doctor’s round.
Exceptions to visiting hours may be made by the charge nurse or primary attending physician according to patient need.
Children under the age of 12 visiting the hospital are to be accompanied by a parent or responsible adult at all times.
We advise visitors to keep conversation and activities in the hallway short. Being quiet outside patients’ rooms is important to ensure the comfort of our patients & families.
We limit visitors to two per patient in a private room.
Beds and bathrooms in general wards are for patients only.
One companion is allowed per patient in the general ward during visiting hours.
If you are in a shared private room, please help your guests respect your roommate’s need for quiet and privacy. This may include loud phone/voice conversations and TV volume.
Overnight visitors are allowed only when the patient has a private room.
For the protection of our patients, after 7 p.m. security officers will check all waiting rooms if visitors are authorized to remain after-hours.
The hospital does not permit children under 12 on patient floors or allow them to be left unattended in the hospital.
Visitors are requested to temporary leave the room during cleaning hours.
The discharge process takes at least 2-3 hours after receiving the intimation from the wards.
Vehicle parking is at the Owner’s risk.
Visitors are requested to take care of their belonging (like Cash, Mobile, Jewellery etc) as the hospital shall not be responsible for the loss of any such personal belongings.
In case of any breakage of hospital property, inform the nurse on duty.

PATIENT PORTAL - Monitor Your Health Records Anytime, Anywhere​​

Following registration at our patient portal, you can view lab results and appointment information online (past and upcoming appointments). ​The patient portal also allows you to reschedule your booked appointment. 

WHEN YOU ARE DISCHARGED

Your physician will determine when you are medically ready to be discharged from the hospital. This doesn’t necessarily mean that you are completely well-it only means that you no longer need hospital services. We ask your cooperation in making transportation arrangements for no later than 11 am on the day of discharge. This allows us to move seriously ill patients from our emergency department to a patient room to facilitate their care. If you need assistance with transportation, we will make arrangements on your behalf with a local taxi service.
